Community Links Worker

Our Community Links workers, Leila and Sam are available here at the surgery to help patients easily access support available within the community for their particular social, emotional and practical needs. The Links team aim to empower patients to make improvements to their health and well-being through informal and confidential one-to-one chats lasting up-to an hour. Through a course of 4 to 6 sessions, they will assist patients to identify their specific support needs and then find suitable specialist agencies within the community that can help them address those needs.

They can help patients access support for issues such as:

1. Social isolation and loneliness;

2. bereavement or loss of a loved one;

3. Homelessness or poor housing;

4. Childhood abuse, trauma, violence or neglect;

5. Education and training;

6. Community engagement and volunteering;

7.Poverty, debt or unemployment;

8. Benefits and welfare rights;

9. Alcohol and Substance Use;

10. Poor mental health; and

11. Any other issue affecting a patient’s health and wellbeing.

If you think that you or someone you know would benefit from the links worker service, please speak to the reception staff or one of the GPs or Practice Nurses here at the surgery to make an appointment.
