A ten-week course that offers group support, for people living with long term health conditions and/or additional support needs as well as family members to explore and practice self -management strategies that encourage them to live life well.

  • Session 5 - Getting active and motivated
  • Session 6 - pacing and prioritising whats important
  • Session 7 - communication skills
  • Session 8 - the mind-body connection
  • Session 9 -preventing, minimising and recovering from relapse
  • Session 10 - reflection and forward planning
