The District Nursing Team consists of:

The District Nurses are available to give nursing care to patients in their homes and work closely with the GPs. They are based in the Health Centre and can be contacted on 01875 818078. If they are out and about a message can be left on their answering machine.

Thistle Health & Wellbeing Support Assistants

Through wellbeing coaching, courses, community groups and support physical activity, Thistle support people to live well regardless of challenges faced.
Thistle know that it can be difficult to adjust to life with a long-term condition.
Their health and wellbeing teams provide in-person and remote self-management support for people living with long-term health conditions or facing challenging life situations. We're also here for people struggling with Long Covid.
They offer:
- 1:1 wellbeing coaching
- Support to develop a wellbeing plan
- Group-based Lifestyle Management Course
- Moving well physical activity support

Pharmacy Team
Our practice team of pharmacists can provide telephone and face-to-face appointments where needed. The pharmacists, pharmacy technicians or the doctors often review some or all of your medicines and may want to talk with you about the medicines you are currently taking. In the practice we like to review your medicine at least once a year, but can be more often if needed.
